Upon initial contact, an experienced care coordinator will conduct an in-depth screening based on the living situation and care requirements for yourself or loved one. A thorough in-home assessment then takes place which focuses on the personal needs of the individual and with consideration given to the family’s wishes and concerns. After which a customized care plan is created so the cares performed are consistent and complete.
